Use this form if you have a disability or a long term medical condition and you require assistance for your examination. You will need to submit this form at least six weeks prior to your examination.
Please explain the nature of your disability or specific need, including evidence of disability or medical condition. This may be in the form of a medical certificate or other report from a recognised agency, for example, ACC, or in the case of a learning disability, an assessment from Literacy Aotearoa or similar agency.
Please specify what form of assistance you require. For example enlarged print, course work on disk, examination reader, writer or special furniture or access. Please note: An additional 30 minutes of examination time will automatically be available.
